Per Member Per Month Rate Increase for Primary Care Provider Medical Homes

Effective April 1, 2020 – The Per Member Per Month (PMPM) rates for primary care provider medical homes will increase:

• Carolina ACCESS PMPMs will increase from $1.00 to $2.00
• CCNC PCP PMPMs will increase for $2.50 to $5.00 for Non-ABD rates and $5.00 to $10.00 for ABD rates

These rates changes, including reprocessing of previously paid April PMPMs, will be reflected on the May 12, 2020 check sent to providers.
