End-dated HCPCS G0431 and G0434

Claims with HCPCS G0431 and G0434 are being denied. HCPCS G0431 and G0434 were end-dated effective Dec. 31, 2015. These services are currently being reimbursed with an appropriate CPT code. Providers may resubmit these claims selecting the most appropriate code from the lab section of the 2016 CPT manual that describes the service performed. Rates for the new 2016 codes are expected to be added by February 14, 2016.
