Attention: Prescribers New Prior Approval Request Form for Epinephrine Pens

A new fillable, downloadable Prior Approval (PA) form for Epinephrine Pens is now available on the Drug Request Forms page. Until now, providers submitting the request via fax used the standard drug request form. This new form should now be used when submitting a PA for Epinephrine Pens via fax; however, submitting a PA via the secure provider portal is still the preferred method as it allows for quicker and more efficient processing.
