Updates Posted to Pharmacy Prior Approval Clinical Criteria and Forms

Pharmacy Prior Approval (PA) clinical criteria and forms for Juxtapid and Crinone 8% Gel have been added to the NCTracks Provider Portal under the Pharmacy Services webpage. (See link below.) Policy changes have also been posted for the following drug classes: Narcotic Analgesics, Growth Hormones, Triptans, Hemanitics, Sedative Hypnotics, Buprenorphine, A+KIDS, and the Adult Safety with Antipsychotic Prescribing (ASAP) program. In addition, the prior approval clinical criteria for Anticonvulsants, along with the prior approval forms, have been removed.
