Supporting Documentation Requests

When submitting supporting documentation for an individual provider application, documents must be uploaded to the NC Tracks Secure Provider Portal on the Status and Management page. The only exception to this process is requested transcripts, which must be sent in via certified mail or emailed FROM THE SCHOOL in order to be accepted. All documents must be submitted in order to resubmit a returned application.

Please note: The provider cannot submit transcripts to NCTracks. Transcripts MUST come directly from the school by certified mail or email (to be provided by request).
