New NDC for Diabetic Supply Test Strips

NC Medicaid has added a new NDC for Diabetic Supply Test Strips to NCTracks.  NDC #65702071210 is for Accu-Chek Guide 100-ct test strips for HCPCS code A4253, effective Dec. 15, 2018.  The State Maximum Allowable Cost (SMAC) rate is $0.43120 per count ($43.12/box of 100).  This change was authorized by the Durable Medical Equipment (DME) Clinical Policy Unit of the Division of Health Benefits (DHB).
