Attention: Institutional Providers Attending Provider Taxonomy Code on Paper UB04 Claim Forms

There are still some instances in which a provider may submit a paper claim form to NCTracks, such as when requesting an adjustment. Effective with dates of service on or after November 1, 2016, where an attending provider NPI is required to be billed on a UB04 (institutional) paper claim form, it must be submitted with the attending provider’s taxonomy code listed in FL 81b with the B3 qualifier.

Any UB04 paper claim form that is submitted with an attending NPI but without the attending provider taxonomy code will deny with the Explanation of Benefits (EOB) 03101 – THE TAXONOMY CODE FOR THE ATTENDING PROVIDER IS MISSING OR INVALID.
